Australian Primary Care Collaboratives Program (APCCP)
The Australian Primary Care Collaboratives Program (APCCP) supports general practice to make changes in the management of patients with diabetes and coronary heart disease, as well as improving patient access. The three key factors underpinning the success of the collaborative methodology are:
* The rapid improvement cycles known as the Plan-Do-Study-Act (PDSA) cycles
* Moniitoring progress through monthly data measurement and
* The sharing with and learning from others
The APCC National website (http://www.apcc.org.au) contains a wide variety of valuable resources for general practices considering or actively experimenting change or improving practice systems.
