HMR (Home Medicines Review)
As people get older, it’s not unusual for them to be prescribed a number of different medications for various complex and chronic conditions. Sometimes these medicines can react badly with each other causing further illness, even death. Home Medicine Review affords patients with complex illnesses the opportunity to have their pharmacological regimes reviewed in their own homes by a qualified pharmacist.
The HMR program is an Australian Government initiative.
